Vaccine supply is limited and is being offered to patients and community members based on the current Maine CDC guidelines. You must be a Maine resident to be eligible for a vaccine in the State of Maine.
Submit the form below to add yourself to the Martin’s Point COVID-19 Vaccine Waitlist.
Once you are added to our waitlist, we will contact you when it is your turn to schedule a vaccine appointment. If you provide a cell phone number on the form, we will send a text message to schedule an appointment. If not, we will call you using the phone number you provided.
Please respond to the text message or phone call as soon as possible as appointments will fill up fast.
If you are not in the current Maine CDC phase for vaccine distribution, it may be several weeks until we contact you to schedule an appointment. Please check ourWEBSITEto stay up to date on vaccine supply and timing.
If you are a Martin’s Point Health Care patient and you have any questions, please send us a message on theMyMartinsPoint patient portalfor fastest response. We ask that you NOT call your Health Care Center to ask about vaccine availability or updates, this will allow our call center to serve those patients with immediate health needs.
If you do not have a Martin’s Point primary care provider, please direct any questions regarding your vaccination to your own PCP.
Due to the need for careful monitoring of vaccines, we are currently ONLY offering vaccines during scheduled appointments (no WALK-INS) at clinics held at 895 Washington Avenue in Portland. We are not able to offer clinics in other locations and will not be providing vaccines during office visits with your primary care provider (PCP) office at this time.
Martin's Point is only vaccinating those 18 and older due to vaccine allotment.
